I have a foot switch with a 6.3 mm plug. It is connected via a 6.3 mm female–to–3.5 mm male adapter (tip and ring), then to a 3.5 mm–to–RCA adapter, which goes to the PTT input on my Flex 8400.
The foot switch works and the radio goes into transmit, but the microphone does not work when I use the foot switch.
Is there a specific setting that needs to be configured in the radio? What might I be missing?

How is your mic input wired?

Hardware PTT does not work when using the "PC" Mic.

Hardware PTT on the Flex defaults the Mic input to the last Hardware Mic input that was used.

This is something that we're taking another look at (issue tracked as SMART-9340). The original design was made to ensure that when you plug in the hand mic and squeeze the PTT, it would use the associated microphone (the one in your hand). Since then (2013?), we have released a number of features that allow the user to source their microphone input from different places (PC, iPhone, Mac). It is much more common to find that a user has their microphone plugged into a device other than their radio these days.

As such, we are planning to change this behavior in a future release to keep this from being the nagging issue that it is today, especially for new users. Our current plan is to simply leave the microphone input selection alone, no matter which PTT is used. This will mean that you may have to reach over and select Mic from the Input Source selection list if you decide to test the hand mic that the radio ships with. But the number of folks this trips up will likely be less than those tripping on the automatic behavior that addresses it.

So good that you are looking into this. I only use SmartSDR, not Maestro, so a PTT function that works regardless of which program is active on the computer is a very important feature.
