Pull up resistor connection for AG2x8 - BCD Input

Can anyone explain how to assemble a wire harness, or breakout board, with +12VDC pull up, for the AG8x2 BCD input?

This is for connecting a non-com radio to the AG. The radio in question is an Apache Labs ANAN G2 with a DB9 open collector output.
