Trade In Form

I see that there is a trade in form that one has to fill out if they want to trade a radio in towards a new Flexradio. I am looking on the site and I am unable to locate this form. Would someone please let me know where I can easily find this form. I am thinking of possibly trading in a Kenwood TS-990 towards a new flexradio. Thanks!

    I believe the route is to contact the sales people at flex. As i remember, the offer was for residents of the US.

    look on main page at bottom, you'll see purchasing programs, click and on next page you'll find form under trade in your gear!
