It turns out everyone's 6300, 6400, and 6600 radios have the same problem: when using the Flex-6500 with a PA, regardless of whether I use an ACOM 1200S or ACOM A2100, and others using other PA models also experience the same issue—a failure to switch from TX to RX. ... my 6500, 6400 and my current 6600 do(did) that too... Sometimes it doesn't happen for days or weeks, then suddenly it happens again out of the blue – it doesn't matter whether it's CW or SSB – the only thing that helps is to press the PTT and it works again! And as you described, this only happens when operating with a PA (in my case, for example, with four different brands, regardless of whether solid-state or tube). Different settings for CW delay/PTT delay/TX delay did not change or improve this behaviour, nor did various SSDR updates – it is what it is – FLEXRADIO... :-)
Oh yes, I have not (had) this ‘problem’ with any other TRX (brand).
Unfortunately, I can't help you with this, but I can confirm that this ‘bug’ really does exist!
